Paperwork Required for Registration Waiver and Release Form Camp Medical Info Form 1 – Physical taken within the last year must also be attached. School physicals accepted. Copy of Insurance Card
Wee Kicks and Day Camp participants may complete Camp Medical Info Form 2 in place of Camp Medical Info Form 1.
