Steps to complete KU Soccer Camp mandatory medical forms (online):

1. Upload the following documents (A & B) to your computer: 

A) For campers in High School (9-12^th), you will need a current Physical Exam form that has been completed within 12 months of the clinic start date – no exceptions!  OR for campers in K-8th grade, you will need the K-8th Grade Physician Form.

B) Front/Back Copy of Health Insurance Card.

2. CLICK HERE to complete the online medical forms.  You will attach the files above at this time. 

Your registration won't be fully complete until we have received all medical documents.
